Next, an action plan should be implemented to prevent recurrence of the NCE. The degree of action should be proportionate with the magnitude of the problem and the amount of risk encountered. Sometimes a correction plan may be a simple fix, and other times it may entail a complete overhaul of a time-honored procedure. To implement the action, be sure to plan carefully (rather than hastily) when changing procedures and behaviors. Change needs to be managed effectively to be successful. Since the problem was investigated and analyzed with such care, it would be counterproductive to be excessively broad in scope when formulating an action plan. Examine the fishbone diagrams or process maps that may have been used in the analysis of the problem to find specific steps where a change can make an impact.
