The collector MUST complete all the information requested in the collector's portion of the chain of custody by printing his or her name and signing where indicated, and recording the date and time of the collection. If the collector overlooks printing his or her name (First, MI, Last) AND signature where indicated it is a fatal flaw and the specimen will not be examined. The collector must also remember to enter the specific name of the delivery or courier service transferring the specimen to the testing facility.
