Completing the Federal Drug Testing Custody and Control Form (CCF)

Over the next couple of pages, it would be helpful (though not necessary) for you to have a copy of both the Federally Regulated and Non-Federally Regulated Drug Testing CCFs for review. A PDF of the Federally Regulated Drug Testing CCF (approved August 8, 2017) can be obtained by clicking on the icon at the bottom of this page. The form is also available here.
As a collector, you will want to make sure that all documentation is accurate and complete. This means that all required information is recorded on the appropriate custody and control form.
First, check the control numbers at the top of the CCF and on the security strips at the bottom of the form. These must be the same. If they are not identical or are missing, obtain a new CCF for the collection. You will see more about this later.
IMPORTANT NOTICE:
On August 18, 2020 the Office of Management and Budget approved a revision of the current CCF. Employers, collectors, laboratories, and Medical Review Officers are authorized to use the revised CCF beginning September 1, 2020 but may choose to continue using the old CCF until August 30, 2021. After August 30, 2021 employers, collectors, laboratories, and Medical Review Officers will be required to use the revised CCF. If the old CCF is continued to be used until August 30, 2021 a memorandum for the record (MFR) is not required. However, if the old CCF is used after August 30, 2021 then a MFR must be completed. The revision was made to accommodate the use of oral fluid specimens for the drug testing program.
