Cytospin Technique

In the cytospin procedure, use a high speed centrifuge to concentrate the cells on a slide in a uniform monolayer 6 mm in diameter. The monolayer distribution enhances the morphological appearance of the cells present.Allow the slides to dry in air for several minutes and then stain them with Wright-Giemsa stain. Cytospin slides may be placed in an automatic stainer, such as Hema-Tek, or stained manually.Perform a 100 or 200 cell differential and record the number of neutrophils, eosinophils, basophils, lymphocytes, monocytes, macrophages, and blasts cells.Pathologists must review any slide which has tumor cells, unidentified cells, or immature stages of cells, such as blasts.Since criteria for review may vary from one laboratory to another, be sure to check the requirements in your laboratory before reporting the differential.

Pia Arachnoid Mesothelial Cells (continued)

A reactive pia arachnoid mesothelial cell as noted by the darker cytoplasm is present in this field. Reactive cells are a common finding in cytospin smears from spinal fluid samples and are sometimes difficult to distinguish from tumor cells. Mesothelial cells are usually interspersed among the other cells, rather than appearing in clumps. They have a single distinct nuclei that may be eccentric. The macrophages (histiocytes) are seen next to the mesothelial cell. Macrophages are distinguished from circulating monocytes by the irregular appearing cytoplasm. Bacteria, red cells or other debris can often be seen in the cytoplasm of macrophages.

Tissue Cells

Malignant Cells

Malignant cells that have broken away from a tumor within the brain or meninges may also be present in spinal fluid. Tumor cells may be difficult to distinguish from macrophages or pia arachnoid mesothelial cells. While blasts in the CSF also indicate malignancy, in particular leukemia, for the purposes of this discussion, they are considered separately.

Malignant Cells

Malignant cells that have broken away from tumors located in other areas of the body may be seen in spinal fluid. All of the cells in this field are tumor cells. The cells in this slide are characterized by an open, loose chromatin pattern, nucleoli and vacuoles. Notice that the vacuoles are present in both the nucleus and the cytoplasm. Vacuoles in the nucleus are an unusual finding even in tumor cells. Tumor cells are often found in clumps and may have more than one nucleus due to their erratic mitotic patterns. Malignant cells sometimes have an irregular nuclear shape. Bizarre granules may be found in malignant cells but are absent in mesothelial cells.

Pia Arachnoid Mesothelial Cells

Pia arachnoid mesothelial (PAM) cells are often found in spinal fluid because they line the arachnoid space between the skull and the cerebrum. Ependymal cells which line the ventricles may also be present. Since ependymal and mesothelial cells are normal findings, the term mesothelial is sometimes used as a general term for both. It is essential to differentiate these tissue cells from blast cells or tumor cells which have diagnostic significance. An example of a pia arachnoid mesothelial cell, as indicated by the arrow, is present in this slide. Differentiating characteristics of this type of cell include an off-center, single, round nucleus and irregular cytoplasm that exhibits pseudopods.

Chemical Screening of Urine by Reagent Strip
Clinical Significance

No blood is found in the urine of healthy individuals although samples from menstruating females, frequently, but not always, test positive for blood. Hematuria is associated with renal or genital urinary disorders in which the bleeding is the result of irritation to the involved organs or trauma. Examples include renal calculi, pyelonephritis, glomerulonephritis, tumors, trauma or exposure to toxic chemicals or drugs and/or strenuous exercise. Hemoglobinuria may be due to the lysis of red cells within the urinary tract. If it is caused by intravascular hemolysis, the hemoglobin is then filtered through the glomeruli. In the normal individual, the hemoglobin molecule attaches to haptoglobin and in this way bypasses the kidney filtration system. When the hemoglobin/haptoglobin system is overwhelmed, as in cases of hemolytic anemia, severe burns, transfusion reaction, infection or strenuous exercise, hemoglobin passes into the urine.

These cells tend to occur in tight clusters. They may have prominent nucleoli, immature chromatin, and scant cytoplasm.View Page
Osteoblast

Another example of a cell rarely seen in the bone marrow is an osteoblast. Osteoblasts are cells which are similar in appearance but somewhat larger than plasma cells or tumor cells. The nucleus is eccentric and the "hoff" area is sometimes located away from the nucleus. The cytoplasm appears rather foamy when compared to a plasma cell. The size of an osteoblast is 20-25 microns. Osteoblast produce bone.

Mott Cell

Another rare but abnormal type of plasma cell is the Mott cell (morula cell). The compartments visible in the cytoplasm are immunoglobulins which have not been released. Mott cells may be seen in parasitic infections and malignant tumors.

N:C Ratio - Nuclear: cytoplasmic Ratio - The ratio of nuclear volume to cytoplasmic volume within any one cell.Neoplasm - Any new and abnormal growth, such as a tumor.Neutrophilic Granules - Specific granules present in the cytoplasm of neutrophils. Other Factors Affecting Drug Absorption and Distribution

In addition to protein availability, other factors may affect drug absorption and distribution in the body as a whole or at specific sites within the body. Renal Epithelial Fragments

Renal epithelial fragments of collecting duct origin are composed of three or more cuboidal cells. These fragments indicate a more severe form of renal tubule injury with basement membrane disruption. Proximal and distal convoluted tubule renal epithelial cells are not found in fragment form. In addition to the indication of severe tubule damage, proper identification of these fragments is important to avoid a false positive diagnosis of low-grade transitional cell carcinoma. Transitional cell carcinoma is a type of cancer seen in 71% of cases of malignant tumors of the ureter.

Variations in White Cell Morphology - Granulocytes
Barr Body

A Barr body appears as a small drumstick-like projection on one of the lobes of a some of the neutrophil in females. Barr bodies are attached to the nuclear lobe by a single narrow stalk which distinguishes them from other thicker projections, sometimes referred to as "clubs." Clubs have a thicker, and sometimes, a double stalk. This projection can be seen in both males and females and has no clinical significance. Barr bodies must also be distinguished from hair-like projections sometimes seen in the band form, following irradiation or in patients with a malignant tumor that has metastasized. Since Barr bodies are the morphological expression of the inactivated X chromosome, one Barr body can be seen in up to 3% of the neutrophils on a female's peripheral blood slide. In rare chromosome disorders in which three or more X chromosomes are present, two to three Barr bodies per neutrophil can be seen. Recognition of a Barr body in a neutrophil is important in order to avoid reporting it as abnormal unless two or more per neutrophil are seen.
