Category A Packages - Shipper's Declaration For Dangerous Goods

A "shipper's declaration for dangerous goods" form must be filled out for category A packages, but is NOT required for category B or exempt packages.

The shipper's declaration must be handed to the courier picking up the package; do not place it inside the box. The number of copies that are required varies according to shipper, but you should always keep one copy. The shipper must retain a copy of the Shipper's Declaration Form for at least two years. Do not use white out or erasures to make corrections on a Shipper's Declaration Form. If a correction must be made, cross out the incorrect information, using a single line, and sign the correction.

Fed Ex requires that the Dangerous Goods forms be filled out with software using dangerous goods compliance editing checks. This will be discussed further on the following page.

If dry ice is included in a package containing a category A substance, it should also be listed on the shipper's declaration for dangerous goods. However, it is NOT necessary to fill out this form if dry ice is included in category B or exempt packaging; details about the dry ice are included instead on the air waybill.
